Request Forms
To request access to records or personal information held by us, use the forms below. Please complete the relevant form, attach proof of identity (and authorization if requesting on behalf of another), and submit it to popia@signa.co.za. We will process your request in line with PAIA and POPIA requirements, including any applicable fees.
